I am having some issues with the forms heading

Dear sir, please help me to create form, my issue is while creating form I am having issues with checkbox and dropdown codes, I have create a form where I have used both option (checkbox & dropdown) It appears on form but the title or heading of that field just showing “title” on the form I tried all the things but problem not solve, please check the form here “https://lightshadeinc.com/test-form/” after visit the page just see the bottom fields that’s showing on title instead of heading which i have give on backend. thank you
