How to set up fee for each item?

I need each set up fee to show up separately (or total together) for each item that has a set up fee.
For example:
In this below cart, there are two items,
both of those (item #1 and item#2) would include a
$15 set up fee. So the total set up fees at the bottom should be $30But now show only 15
