How to set custom slug for CPT to include the year and month?

My site’s permalink structure does *not* include the date. I’d like to create a new CPT, and only this CPT will include date info in the permalink, ex:
permalink structure: CPT-name/%year%/%monthnum%/%postname%

Where/How can I set this using CPT UI?
