How to removed image placeholder?

    Hi and thanks for the super useful plugin.

    I’m having the same issue about placeholder with 3.7.1 and 2.3.2.

    Even unable to delete placeholder images.

    When a new one is uploaded (and works fine)and then deleted, the placeholder appears again.

    It would be great to find the way to fix it.
