how to modify simple template

hi, we have a requirement to add a signature checkbox to the checkout page and to achieve this we have edited the /templates/simple/packing-slip.php and im hoping to get this edit into a safe place from updates. Can we create a folder in our theme to achieve this and if so what folder structure do you suggest? thank you!!!!
