    I’m not sure of any free ones off the bat, but there are a number of form plugins like Gravity or Ninja forms that either come with this functionality built in or you can purchase extensions for conditional logic. Those two for example, can then be tied in to WooCommerce through one of their extensions if needed.

    If something like that doesn’t suit then the google search terms “WordPress Conditional Logic Form” might be your best bet ??
