How to make my form automatically include name of the page in email?

Hi there,
We have dozen event pages, on each page we need a booking form.
To avoid asking people fill in the name of the event, would be cool set the form in a way that when we get email we see what event page this form has been sent from.
In other words – Is the a way to make form automatically include name of the page the request has been sent from?
