How to make a custom field required using DIY HTML web form?

I added a Get Response web form using the DIY HTML web form code provided here: https://support.getresponse.com/uploads/2013/11/DIY-WebForm.pdf.How do I make all the fields required so I can see what people entered when they submitted the form?
As an alternative, is there a way to create a 2 column web form using the plugin?
