How to Integrate default values with website's database

Hello! I’ve created a form using this plugin. I just would like to know if it is possible to integrate the fields’ default values with the current logged in user’s information stored in the website’s database. Like for example, I created an Order Cancellation Form for my customers. I want that the fields’ default values for the Name, Email, and Contact #, will be the ones stored in the database. Most importantly, the Order #(which can be found in the invoice that was sent to the customer upon checkout) field should match with the one stored in the database so as to validate the cancellation request.
