How to insert the captcha on custom page?

I love this plugin, but i want to insert it into custom page, especially “WP Maintenance Mode”. When Maintenance Mode is ON, there is a “contact form” in the main page, to contact with admin, i need to add captcha to it…
If anyone could tell me how it will be great, thank you!
