HOW to Insert extra fields in form

  • Hi everyone.
    My issue is as follows:

    i use cforms plugin to make registration form for users . I would like to create a list of categories in the way that each person could pick which is his “favourite” – during registration, or later. So – I need to have as a result categories saved in database connected with user.
    Any help how to add this?

    and by categories of course i dont mean wordpress “categories”, just- organized items (picture + text, for ex. Art, or Politics – saved with a pic and a name – organized in parent-child way…- 1.Art-1.1. Acting,.. 2. Politics – 2.1. International P., …) – being able also to add categories
