Whilst you are on, do you know if a two colum form will be correctly displayed on a smartphone?

Or is it better to stick with a long, single column form?

Assuming you are using a responsive theme they would usually be rendered as a single column on narrow devices. There are plenty of free tools you can use to test how the page will render:
