how to have placeholder text dissappear on focus

On this site:
37.60.244.202/~healt984/ the form near the top of the home page (in a header image), when I enter text in the form text fields the placeholder text does not disappear. As I look at your plugin code it looks like it is supposed to? I tried a couple of on-line fixes with JQuery and css but no luck.
