    I found a solution myself after reading code of plugin

    The shortcode to display the front end form is:

    [lava_directory_form]

    The shortcode to display the directory listings is:

    [lava_directory_listing]

    The shortcode to display “My Page” is:

    [lava_directory_mypage]
