How to create a custom register form for login

Hi IT pro, i need someone who have a good understanding on wordpress to help me achieve this. i builded a website and i need the users to be able login and fill information like Name, Address, Sex, phone number and probably picture box. glad to hear from someone to guide on how to go about this.
