How to configure the Author Information Widget?

In the first section of the Author Information Widget (Form Type) I entered Contact Form and then the page ID of the contact form. I saved changes and hoped to see a button, a link or the contact form in the Author Information Widget. Nothing changed. I still see the email address displayed to the public.
What is the right answer that would NOT display the email address to the public?
What is a Report Type in the second part of the Author Information Widget?
