how to add report post button?

    I want to add report post button for my selected post. I will appreciate a plugin recommendation that provides me with following features:

    Adds report button to certain posts.

    Only logged on users can report.

    Admin can see the reported post with user details whoever reported on admin screen.

    Has a problem description form for users.

    Adds a threshold limit and after threshold limit is reached it automatically hides the post and keeps it for moderation.

    I have searched many plugins but none of them served my purpose and most of them are out of date. A help will be really appropriated. I have also checked other answers here but neither one helped.
