How to Add more Input Fields?

    Hello! Love the plugin so far! Two things I need help on.

    1) Can you tell me how I can add more Input Fields? Currently, I am able to set up 5 fields but I need more.

    2) When I updated to the new version today, the plugin is not visible anywhere in the wp-admin. I checked my FTP and the plugin is in there…. just not showing up anywhere on the dashboard.

    But yeah, If you can tell me how I can add more Input Fields, that would be the best!!!!!!
