how to add custom form

Hello every one
i’am new to WordPress,i want some information about form.i create a web site for my business purpose.in my website people put some entry via my particular page the entry fields are like name,age..etc.when people hit save button the fields values are stored in database after that i’am only show that details in a particular page or dashboard.if it is possible and how can i achieve this.
