How to add custom fields & Text to form.?

Hi,
Can anyone please tell how can I add custom fields to the form.? Also, I tried editing plugin core file: seamless-donations/dgx-donate-paypalstd.php to add some line of text above the Donate button, but its not showing at all. Please help..!
