How to add custom fields addresses to WooCommerce product?

If I understand you correctly, you can create the fields with WooCommerce core.

Let’s say the Medical Professional is the “Product”. With a variable product, you will be able to add the services as an “Attribute” and the address will be the “Attribute Value”. Just like a T-shirt is the product, the color is an attribute and the size the attribute value.
Get to know more about variable products here.

However, the best solution to add all the required steps that you want to provide to your customer is the plugin WooCommerce Bookings:

This plugin allows customers to book appointments and can sync with Google Calendar to show availability and avoid double bookings.

And I would like that these fields can be Autocompleted with Google Places API.

I’m not sure if I understood this part. Will the autocomplete be available to customers or just in the backend?

As you advise I will try to use Attribute and Variations.
But the fact is that in my system I consider Medical Professionnal as WordPress User and not as WooCoommerce product. So the service is an WooCommerce product and it’s link to an user (with the field ??staff?? added with the plugin WooCommerce Appointment).

Concerning the use of the plugin WooCommerce Bookings, as I said in the last sentence I already use the plugin WooCommerce Appointment. So I don’t know if the two plugins do the same job?

@csnlima the autocomplete fields will be fill in the admin and the value stored will be show on the appointment form.

From what I am understanding:

Products = services
Medical professional = user

If this is the case, what would work for this is a multivendor plugin. You can achieve this with a plugin such as:
