If you’re using a plugin to add custom fields to your Woo registration form as mentioned by @kaavyaiyer, you could ask the plugin author if there is a feature to upload an image, otherwise check out this tutorial from Business Bloomer:

As a side note, I would recommend using a plugin like Code Snippets to add custom PHP code into your site without directly accessing the functions.php file. Here is a link on how to use the Code Snippets plugin:
