How on earth do we use this?

When you install it and go to the settings page you will see a section at the top of the page called usage. This talks about how to use the plugin.

The free version has the ability to set a fixed price. However the pro version includes the ability to link form fields to the price and other fields in PayPal. That documentation can be found here:
