    1. Define your form in Google Drive. Make sure to downgrade your Google Form so it will work with the plugin. Get a shareable link from Google for the form. It must be visible to anyone who has the link.
    2. Define your form within WordPress. You use the URL to your Google Form defined above to create a WordPress representation of your form.
    3. On your WordPress Dashboard look for “Google Forms” and “Add Form” (Dashboard > Google Forms > Add Form).
    4. There are lots of options but you only need two in order to define a form in order to test it: Title and URL Everything is optional depending on what else you want to do with your form.
    5. Publish the Form
    6. From Dashboard > Google Forms you will see a list of forms you have defined. You should have one and one of the columns will have the short code syntax you can use in a post or page. You can also use the “View Google Form” link after the form has been pubslihed to see theform using the permalink URL.

    These steps outline the two minute jump start to get a form published. When you first publish the form it may not look very good which is where custom CSS comes into play. That is an advanced topic which there is no point covering until your form is working correctly.

    Thanks Mike, So far so good!
    One clarification for anyone else:
    ON Step 2 ” Get a shareable link from Google for the form.”
    I used the link from the form’s menu which has a Globe&Link icon, as opposed to the “Send Form” button at the top right corner. The Globe&Link url does not have “?usp=send_form” at the end. Not sure if that makes a difference, but it worked.
    The permalink works and I’m going to try installing the shortcode in the UX Builder now.

    Either form of the link will work. I usually drop the ?usp=send form portion of the link because it isn’t necessary.
