How do I specify which pages the subscribe form is on?

    Since updating the plugin, I cannot find the setting to specify which page and placement of the signup form?

    At the moment it is showing on my main blog page (but it is above the posts) – I want to put it at the bottom of the page.

    When I first installed the plugin, the form used to be at the bottom of the page.
