How do I remove Contact Form?

I have installed the indeed plugin on our wordpress web site. After I post an ad, I created a page and put “[indeed-jobs]” in it. When I opened the web page to look at the ad, I found what I posted. When I clicked on the ads, they opened up but I got a surprise. Beside the ad, “Contact Form” showed up. Under the page holding the “[indeed-jobs]”, I set the template to full width. I want the subsequent ads to do the same. There are no additional windows I can change, so it has to be something in the plugin. How do I set the ad window to full width?
