How do I install a site wide form such as a shaker box?

Hi, I created a shaker box in my get response account and have the code to install it. I also have the updated version of this plugin installed in my website. However, when I review the the plugin in my site, it gives me no method to install a site wide form such as a shakerbox. Before you say to install it in the Header (after header) (before body), etc, I am using the theme Customizr – child theme. That theme does not allow for inserts into the header area. They have a place to add css style sheets but I don’t know how to do that. I also cannot find instructions on the getresponse website on how to install it either. I spoke to chat from within my account and they told me I should come to wordpress to get instructions. So can you please help me with this?
