How do I get Twenty Seventeen to recognise Pirateforms?

    I am setting up a site using the Twenty Seventeen theme. I have activated pirate forms, filled out all the relevant details, made sure it is down loaded and activated etc. How do I get it onto the actual “Contacts” Page?
    There is no link in “Edit Page” page to assist with this and no link, under pirate forms or plugins that explains how to do this.
