How do I get different texts with a purchase?

Hi,

I have a (for me hard) issue.

I am selling a shoe tag (soscoin) for runners. At the tag i can print information about the runner what can be used in a emmergency.

At another website i created a pricing table, try to sell 1, 3 or 6 soscoins.

My issue is how to get the texts from the buyers? Now i installed a Flexibel Product Fields plug-in, and it works for 1 product. I think it works also for 3, but for 6 divided over 2 products it is a problem, because i do not know which field is for which product.

Maybe you suggest another way of getting texts. My goal is to sell “Brons” for e12,50 and let the customer fill out 1 form. For “Zilver” the package deal cost e25 and the customer must fill out 3 forms. For “Gold” the customer pay e75 and he need to fill out 3 forms for 2-holes soscoin and 3 forms for a 4-holes soscoin.
