How create a form , where can we find Shortcode

  • Hello, i d’ont find anything to use this plugin “Cost Calculator by AZEXO”
    How create a form , where can we find code or Shortcode..etc..
    i find some code using your content editor azeo and then coming back to html code but if you have some code list it will be nice.
