How can I make selectable options and mail those selections?

The user has to make X amount of options to choose from, after choosing those options he clicks on the ‘send button’ which forwards his selected boxes and mails those forward to someone from who will contact the user.
What’s the best way to handle this in WordPress? I’m a complete newbie here.
I’ve made a simple example using ‘cars’, there are probably other input fields above where the user leaves his credentials. I am using a basic free theme.
