How Can i Create a form like this on wordpress

Almost any form plugin will do. I’d choose formidable as (1) it’s my favorite and (2) I’ve been using it for a long time so I know how to make it work.

I want a system where users can login and fill a form on my wordpress site, then make payment. admin then approve their form and they can print out to use it as a gate pass or ticket for an event.

QR codes work well for event tickets, just saying ??
That would (at minimum) require integrating a couple of plugins though, so the feasibility depends on your technical skill.
