How can I add a credit card payment to the form?

Hello, can somebody please help me with a little advice, I want to add a contact form with payment gateway on my website, to receive the details from the form and payment confirmation, so I can send to the customers their desired documents, I attached an image to show exactly what kind of form I want, is the one from the right that was rounded with the pen, thanks in advance.
