Honeypot – Form reformatting

Hi there, I am in no way tech savvy but the form on my page keeps coming up with a “are you human” capcha and the forms looks strange. When I refresh it dissapears.

I thought the update fixed this honeypot issue?

Any advice on how I can fix it? Meant to be going live soon!
