Hide last name

No problem! To do this, open your form builder and click on the existing Name field. This will open a Field Options screen. There, you should see a Format dropdown (shown in this screenshot: https://cloud.supportally.com/0S3m0Y2n1l3H). Select ‘Simple’ and the Name field should become one big field (no first name/last name).
