Hemingway Theme – Extra HTML Tags

Hi – Great Idea, but having trouble getting it to work on Anders Noren’s Hemingway theme.
When I activate the plugin and add a subtitle, it ads extra markup to the output. and repeats the Title and Subtitle.Screenshot of issue here : https://db.tt/ML6p04Y8
Am attaching a screenshot as I have disabled the plugin on the site currently. My site is asabharwal.com
