Help with setting you Mail section

I am new to ContactForm 7 and would like some initial help.
I have been asked to create a form that has three doctors to select and this should send an email to that selected doctor.
I believe the form part is correct as I followed a tutorial. See below:

Now, I can’t seem to get the To and From elements of the Mail set up to accept the email address from the person sending the form [eMail*] in the case, and more the option to select the doctor’s email to place in To:
Can anyone help me understand how I can put the form tags in the Mail fields to have it populate with the necessary informations?
