Help: How to add survey/questionnaire before checkout on E-commerce site

  • Hello. I’m working on an e-commerce WordPress site for a physicians. They will be selling multiple products, but a handful may require the customer to fill out a survey or questionnaire before purchasing. There will be no right or wrong answer to the survey, but it’s just information that we need to collect if this person is purchasing these products.

    Does anyone have tips on how to best accomplish this? The tricky part is that not all products will require this survey. Only a few.

    Just had a thought about finding some kind of survey that will appear in a popup on top of certain pages via shortcode. That might do the trick!
