Help getting html form displaying proper on page

Hello, I’ve seen the wordpress community is one of the most responsive communities around so I thought I’d ask my question here. I’ve given all details I think are relevant, but if I’ve left something out, please ask me the question. i feel there is probably an easy fix for some who is more knowledgeable with wordpress. Thanks in advance for your time and help. Ok here goes:

When inserting the html code I click on the html button on the page I’m editing. I’m not using the visual editor to insert the code

I have added the same html code into a text widget located in the bottom footer space and it displays closer to what it should look like.

If I add the code to an html page in dreamweaver it displays correctly.

From what I’ve learned searching the forum, there seems to be some security measures WordPress uses to protect you from html that contains form tags. I need to disable that feature for wp pages
Any help is greatly appreciated. Let me know if I left something out.
