Form plugin

Hello,

I’m looking to recreate functionality in WordPress and need a plugin to do it.
Here’s what it should do.

1. Visitor visits page and fills out form, simple name/address form.
2. Submitted data is immediately made available in a downloadable list in CSV/XLSX format.

This is a simple “do not knock” registration. Residents of a city add themselves to a list stating they do not want soliciting and vendors can download the list.

Any addon that could do this?
