  • Maybe you need to add a example ticket for try if the form work. And Title of what? You can edit po file for change the original title showed on front page.

    Order Form Instructions can be set either as an attribute, or on the Options page as of the latest version. The instructions set as an attribute take priority.

    The form title can be edited using this attribute:

    order_form_title
