When you create page or post you can see right side of text editor SMART FORM icon. Press icon and choose form. Finish.

Good super early morning Edgar. Well, no, numbers did not just show up in place of the check boxes. When I typed that message in, I scattered the number one to the far left, then 2 on the following line a bit more to the right, and so one. DOn’t know why WP here put them all in a line. Originally the check boxes looked like a set of stair cases with regards to how they were placed on the page. They looked normal, but it was how they were placed. that’s all.

ooh, ok, do you have an screen shot, or an url that i can check which is showing this behavior? i wasn’t able to replicate this in my test envyroment

I tried to use this plugin, I created the form and placed it on the page but nothing is happening when I click on the submit button.

Yes please, i don’t know how much that will help though, since i think i that i would need to check the javascript part.

I tried again, and now the form started working fine. I don’t know where was the issue. But here is something I would like to know first is how can I give the Reset Button and how to make a constraint on field checking like email and mobile number checking (about their formats).
