Header is coming twice in a webpage

I am facing a problem that the header in my website is coming twice. I dont know how it has came. i have checked the following but no luck with that:

1. Checked all the code for the duplication of header image.
2. Checked the css file, if there is any duplication there.
3. Changed the image to some other image but still it is duplicating.
4. Change the place of the code ( like i have put it outside the div)
But no luck

Is there any php code which is duplicating it or something???
