Has changed image name

It is the Cache-Busting option in the settings page.
You have to disable it.
If you are using Stateless mode then you have to use WP_STATELESS_MEDIA_CACHE_BUSTING constant to do so.

No, it will only work for new uploads.

To achieve that you have to write a custom PHP script.
That will rename the file and update the metadata accordingly.
