Guestbook Help Needed

I attempted to make my own page and create a Guestbook based on the instructions from this site https://geeksmakemehot.com/wordpress-guestbook/ .Everything is done except on the Guestbook page there is no text area for people to leave comments… just a blank page basically.You can click the link below to see a screenshot of the problem.
