I have purchased mtouch premium plugin

I have enabled Gravity Form

When I include mTouch Quiz on page https://www.gointerviews.com/quiz/

But when I include the same quiz on page https://imgur.com/YV5Jw4Z

I do not see the gravity form

Please resolve
