Google Custom Search Issues

I have seen a few other posts with people having problems with Google Custom Search, however I have not seen any fixes for me.

I have tried putting the code into a template, and directing the search to that published page.

I have also tried putting the code directly onto a page (html view).

When I try to do a search, I get blank page.
I have waited 5 days for Google to crawl, issue persists.

I am thinking, is it possible that Google Custom Search doesn’t like some javascript on my page or in my header?
