Go to pre-purchase form first?

We have one category of products for which we need the customer to fill out a short form prior to selecting and purchasing their products, which means that they need to see this form before they get to the product page. We have the shop page set to display categories, and was wondering if there is a way to have just this one category link to the form, which then when the submit button is clicked, forwards the customer to the product page. I already have the form set up and functional, but I need to have the image for the category linked to the form instead of the product page, but just for this one category. Would anyone know how I could accomplish this? Thank you.
