Glitch with certain fields

I’m finding that on my booking form, created with Contact Form 7, that a couple of fields have glitches: https://www.westcoast-mountainguides.co.uk/booking-form/
1) Course Dates: this field always comes back as if nothing has been entered, so looks like this on the email: [text course-dates] I’ve tried submitting a form, and entered a date.
2) Method of Payment: this field always duplicates the following field, How did you hear about us? So will come back with something like ‘Facebook’ or ‘Word of Mouth’.
Here’s an example of a form I’ve just filled in and the email generated, glitches in bold:
