Gig Guide Form

Hello – I’m pretty new to the WP world and need some help creating a form…

I want to create a Gig Guide for my website that a select few people can update.
It only requires 3 fields – DATE – VENUE – BANDS (simple as that..!!)
BUT – when the inputter clicks submit I want the info to automatically enter a list on my site in the correct date order (ie date of the show, not the date of the submit).
Can anyone help?
